Fees

Comprehensive Chemical Health Assessment:  $375 - flat fee

Includes on-line assessment tool (SASSI); personal interview (90-120 minutes); 3rd party collateral phone interviews; written report including recommendations and referrals; follow up to assist in connecting with resources as needed and to discuss recommendations with attorneys, probation officers, trustees, parents and guardians.

SAP Evaluation Process:  $425 - basic fee

The SAP evaluation process meets the minimum requirements set by the Department of Transportation.  Includes online assessment tool (SASSI); personal interview (90-120 minutes and follow-up session following completion of required activities; consultation with employer; 3rd party collateral interviews as needed; written communication with employers regarding requirements for education, treatment, and follow-up testing plan as indicated.  Should additional services be required, they will be billed at the standard session rate of $150/hour.

Individual and Family Counseling:  $160/hour

There is no additional charge for meeting with more than one person.

Payment & Insurance

Payment is due at the time of the appointment unless other arrangements are made.  I accept cash and credit cards and do not process insurance claims or payments.

Insurance

I do not take insurance directly. However, some insurance companies may cover all or a portion of my fee using your out-of-network benefits. You may also be able to use your HSA, flexible spending dollars, or medical savings account to be reimbursed for my fees.  If you would like to use your insurance, please contact your insurance company and ask what coverage you have to see an "out-of-network provider." They will tell you what your coverage is; if possible, ask them to fax you or email you a copy of your coverage so that you can use this when you submit any documents to them in the future. Please check your coverage carefully by asking the following questions:

  • Do I have substance use or chemical dependency insurance benefits?

  • What is my deductible and has it been met?

  • What services does my health insurance cover?  How much does it cover?

  • Is approval required from my primary care physician?

We can begin your care, you will pay for the sessions as we go, and you will be given an itemized bill that you can submit to your insurance company for reimbursement.  I am not able to make any assurances that your insurance company will accept the charges or provide reimbursement.
